Job Description

Takes a Nursing History at admission. Uses resources/information appropriately. Makes pertinent observations. Assesses mental and physical of patients health status per shift. Identifies and prioritizes individual client needs .Prepares a nursing care section of interdisciplinary treatment plan with measurable, realistic behavioral goals based on identified actual and potential client problems. Implements nursing care actions to promote goal attainment. Evaluates effectiveness of nursing actions against stated goals and problems. Revises nursing care plans with changes in patient condition. Understands the roles and functions of other members of the interdisciplinary treatment team. Communicates the role of the psychiatric nurse to the other members of the health team. Participates in planning, implementing and evaluating the treatment plan. Participates in flash meeting and/or treatment plan. Participates in care conferences with client included, as appropriate. Collaborates with members of the health care team. Participates in planning the resolution of ethical dilemmas. Effectively manages unit issues and concerns at the lowest level possible. Provides a safe environment and takes into account the safety needs of the psychiatric patient. Measures vital signs including neuro-vitals. Implements seizure precautions, as appropriate. Performs suctioning technique. Administers oxygen therapy. Assists client with coughing and deep breathing exercises. Maintains established intravenous lines. Inserts naso-gastric tubes. Administers gastric feedings. Feeds Clients when necessary. Assists clients to maintain hygiene. Assists client to maintain normal body functions. Cares for client with drainage e.g urinary. Performs urinary catheterization. Administers enemas and suppositories. Provides ostomy care. Transfers and turns clients. Safely operates mechanical lifts. Ambulates client. Demonstrates proper use of body mechanics. Follows procedures to minimize risk of decubitus ulcers. Carries out isolation techniques. Performs sterile dressing techniques. Applies dressings wet and dry. Applies hot and cold compresses. Provides cast care. Obtains specimens (except venous and arterial blood, and vaginal specimens) Tests capillary blood glucose level. Follows policy on admission, transfer and discharge procedurePrepares client for diagnostic procedures. Cares for the body after death Administers oral, rectal, vaginal, topical, intramuscular, intravenous, and subcutaneous medications. Instills eye and ear medications. Performs procedures related to specific drugs. Follows control drug regulations. Assesses need for PRN medication. Assesses and recognizes the effects and side effects of drug. Follows established procedures for safe medication administration. Uses equipment and materials to provide support and protection, e.g. Restraints. Operates wheelchairs, gurneys, portable commodes, etc. Ensures safe equipment is available for the client. Adheres to Universal Precautions..
