Job Description

Responsibilities In collaboration with the care managers/manager/director of the Medical Quality Group, the Quality Improvement Analyst is responsible for assisting in the coordination, integration, and implementation of the primary care clinic wide performance improvement activities to ensure compliance with company policies and state/federal regulatory, contractual, and payer standards. Duties include but are not limited to chart reviews, data abstraction for CMS Core Measures, payer measures, HEIDS, STARS, and other quality outcomes tasks, and staff education regarding core measures and updates regarding new and enhance requirements and standards. Serves in an expanded role to collaborate with specialists, health care team members, to ensure appropriate follow up transitions are in place for transitional care management.

Education and Work Experience
1. Two years’ experience with adult medicine, primary care experience/ambulatory care experience
2. Bachelor’s Degree in healthcare related field preferred.
3. Healthcare Quality experience preferred.
4. Excellent computer skills are required including proficiency in (MS Office Word, Excel, Access).
5. Experience in reviewing charts for core measures.
6. Detail oriented and organizational skills.
7. Must be able to handle multiple tasks, directions and be able to follow-through.
8. Good communication skills both verbal and written.
9. Experience with Medical Staff communication a must.
10. Excellent interpersonal relationship skills with exceptional professional work ethics.
11. Proficient in word processing, spreadsheets, database and typing.
