Job Description

Overview Provides personal care to patients in accordance with patient’s individual needs, the plan of care and Agency policies and procedures.     

Observes patient status and reports to the Registered Nurse, Clinical Manager/Designee as appropriate.

Participates in Patient Case Conference and contributes to the modification of the plan of care and individualized goals.

Encourages patient/family participation in self care as indicated by patient status.

Contributes data to multidisciplinary Agency professionals for the coordination of patient care.

Participates in orientation of Agency Home Health Aides.

Initiates consultation with Registered Nurse and/or Clinical Manager/Designee as issues and concerns arise.

Demonstrates consistent educational development through in-service education and maintaining certification per Agency policy and procedure.

Participates in Performance Improvement activities, community health screening events, agency committees, staff meetings and other meetings as assigned.

Completes assignments effectively and efficiently.

Completes clinical and payroll documentation and submits documentation for personnel file per Agency policy and procedure.

Practices good guest relations and exhibits the values and mission of the Agency.  Maintains patient/employee/Agency confidentiality.

Performs other duties as assigned.

 

Responsibilities The Nurse Assistant in the Med Surg/Telemetry Department is an unlicensed non-certified assistive person who assists with activities of daily living, provides basic nursing care to non-critical patients and assists in maintenance of a safe environment under the direction and supervision of the Registered Nurse in charge of the team and/or unit.

 

Education and Work Experience

 

1. Current Certified Nursing Assistant certificate upon hire, preferred.

2. Current BLS (AHA) within 30 days of hire and maintain current.

3. Possesses knowledge of Medical Terminology.

4. Minimum six months previous experience in an acute care hospital or physician’s office preferred

 

Facility Specific Requirements (facility may require items listed below):

 

1. Current Behavioral Violence Prevention (BVP) certificate within 6 months of hire and maintain current.
