Responsibilities

JOB SUMMARY: The Lung Health Coordinator, under the direction of Nursing Administration, will navigate patients throughout the lung cancer care continuum to reduce barriers to care and improve the quality of life for patients and families.  The Lung Health Coordinator acts as care coordinator, educator, and advocate to achieve optimal outcomes. The primary task of the Lung Health Coordinator is to decrease fragmented care, coordinate services, and guide patients through the health care system to receive efficient and effective diagnostic and medical treatment services, as well as facilitate their transition back to their primary care physician and community. The Lung Health Coordinator will also provide patient education to assist informed decision-making, improve timeliness of care, and coordinate multidisciplinary care processes and supportive services. The Lung Health Coordinator will be responsible for maintaining a central database/navigation software system to assist with navigation, continuous quality improvement and clinical research. 

 

Serves as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within St. Joseph Medical Center and in the community

Qualifications

EDUCATION, EXPERIENCE, TRAINING

 

  • Valid State Respiratory Care Practitioner (RCP), Licensed Practical Nurse or Registered Nurse License required

  • Current BCLS (AHA) certificate upon hire and maintain current.

  • Minimum of 5 years of acute care or pulmonary experience preferred.

  • NRP certification preferred.

  • PALS certification upon hire and maintain current

  • ACLS certification upon hire and maintain current

  • 7. Ability to communicate effectively, timely, and in an open, honest manner.
