Job Description

Overview

This position will be located in the St. Joseph Family Medical Care Practice.  On the St. Joseph campus.
Responsibilities

Provides care management and care coordination for adult and pediatric patients with 3 or more chronic conditions in the primary care setting.  In partnership with the primary care practice leadership team and Medical Group Quality team; the Care Manager provides assistance with training, and delegating to other members of the team.  Serves in an expanded health care role to collaborate with specialists, health care team members, and patients/families to ensure the delivery of care in quality, efficient, and cost-effective health care services.  Responsibilities include but not limited to assessments, care planning, implementation, coordination, monitoring and evaluating all options and services with the goal of optimizing the patient’s health status.  Integrates evidence-based clinical guidelines, preventive guidelines, and protocols in the development of individualized care plans that are patient-centric, promoting quality of care and efficiency in the delivery of health care.   

Manages and follows a designated caseload of patients within the associated practice groups.  Provides targeted interventions to avoid hospitalization and emergency room visits. Coordinates care across settings and helps patient/families understand health care options.  
Qualifications

 

  1. Current LPN license in the state of Missouri required
  2. Three years of experience with adult medicine and pediatric patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical setting, within the past five years.
  3. Knowledge of chronic conditions, evidence based guidelines, prevention, wellness, health risk assessment, and patient education.
  4. Critical thinking skills and ability to analyze complex data sets. Ability to manage complex clinical issues utilizing assessment skills and protocols.
  5. Excellent assessment and triage skills. Ability to implement evidence base interventions and protocols for chronic conditions.
  6. Demonstrates excellent communication--both verbal and written.
  7. Excellent interpersonal and facilitation skills.
  8. Ability to affect change, work as a productive and effective team member, and adapt to changing needs/priorities.
  9. Time management, priority setting, work delegation and work organization.
  10. Effective computer skills.
