Responsibilities
Responsible to and under the general direction of the Director of Rehabilitation. Assumes responsibility for screening potential inpatient rehabilitation patients for appropriateness of admission, facilitating admissions from referral sources/agencies, completing all documentation requirements for CMS including prescreens, IRF-PAI data and all other Prospective Payment Systems (PPS) requirements, completing IRF-PAI sends, applying Utilization Review criteria for potential admissions as needed and secures pre-certifications and re-certifications of inpatient rehabilitation admissions The Referral Coordinator works cooperatively with other patient team personnel in maintaining professional standard and interdisciplinary teamwork. The Referral Coordinator participates in marketing activities, performance improvement, program evaluation, program development activities, and maintaining positive relations with all customers. The Referral Coordinator will also participate in special unit functions and marketing activities as assigned by the Director of Rehabilitation and completes site visits to physicians and providers as needed for potential admissions.
EDUCATION, EXPERIENCE, TRAINING
1. Bachelor of Science in a related field from an accredited College/University.
2. A minimum of two years case management or clinical rehabilitation experience preferred.
3. Current BLS (AHA) certificate upon hire and maintain current.
