Job Description

JOB SUMMARY Responsible for the assessment, planning, implementation and evaluation of nursing care provided to patients. Functions as an effective member of an interdisciplinary team in coordinating care to patients. Performance Expectations NURSING/WORK PROCESS Assesses the physiological, psychological, socio-economic and educational needs of each assigned patient. Incorporates OASIS items into appropriate assessment data bases. Formulates and initiates the plan of care based on evaluation and identified patient needs with patient, family, physician and other team members; includes discharge plan. Implements care for each assigned patient reflective of the medical plan of treatment and established care plan. Develops and implements teaching plan in collaboration with patient and family. Evaluates patients response to treatment including progress towards identified goals and revises care plan as indicated. Supervises care assigned to LPNs and Home Health Aides on the patient care team. COMMUNICATION and DOCUMENTATION Communicates effectively with all health team members and customers. Responds in a timely manner to clinical managers or team members regarding patient care issues. Communicates changes in patients condition/needs to physician and other team members as appropriate. Participates in case conferences to assure coordination of care. Documents inter-disciplinary communication. Documents care per agency policy. Meets time frames required by regulatory bodies and agency policy. Confers with supervisor in a timely manner regarding problems, incidents and suspected abuse/neglect. Works in partnership with members of the clinical team to effectively manage patient care. Participates in on-call rotation as designated; demonstrates ability to prioritize patient needs after hours, and responds in a timely manner. Is committed to improving customer satisfaction. Participates in, and supports, efforts to improve organizational performance, including chart audits, performance improvement teams, and PI committee. TECHNICAL/PROFESSIONAL COMPETENCE Reviews new and revised policies on an ongoing basis. Maintains a conscientious appearance in regards to dress code and personal grooming. Attends staff and discipline-specific meetings; review minutes when absent. Participates in competency testing as required. Maintains current professional requirements to include licensure and/or certification, CPR, health assessment and TB testing. Updates knowledge base through attendance at continuing education/in-service offerings, journals, etc. Acts as preceptor for new employees and students as requested.
