Job Description

Responsible for the overall quality, efficiency, and coordination of care for patients within a designated care team. Provides direct patient care when needed. Assists in managing agency referral process. Supervises full and part-time field staff. Provides insurance case management for team patients. Manages the budgetary expenses related to the care of patients , provide oversight and support to clinical staff; ensures quality and completeness of data entered by clinicians, promotes accountability for work performance of all staff; participates in improving organizational performance through identification of areas to monitor, completes data entry as directed/needed, develops corrective plans of action and implements changes; coordinates the educational process of staff through needs assessment, planning, implementation and education to meet identified goals; initiates program development, trains clinical staff as needed; contributes to planning and implementation of goals, policies, procedures and standards of the department; serves as a community representative to promote the department.
