Job Description

Overview The Coordinator is responsible for maintaining the day-to-day operations of the Bariatric/Surgical Weight Loss and Stroke Program, including referral development, marketing and patient guidance. The Coordinator is responsible for the administrative coordination of all clinical and support services for the program. As a member of the interdisciplinary care team, the Coordinator works collaboratively with other members to assure safe and accurate delivery of care.

The Bariatric/Stroke Coordinator assists in center development, managing the accreditation process and ensuring continuous compliance with MBSAQIP and The Joint Commission Accreditation requirements, maintaining relevant policies and procedures, patient education, outcomes data collection, quality improvement efforts, and education of relevant staff in the various aspects of bariatric surgery and stroke patients with focus on patient safety. The Coordinator supports the development of written protocols and education of nurses detailing the rapid communication and basic response to critical vital signs that is specifically required to minimize delays in the diagnosis and treatment of serious adverse events.

The Coordinator serves as the liaison between the facility and all surgeons performing metabolic and bariatric surgery at the facility and any general surgeons providing call coverage. The Coordinator assists in maintaining the documentation of the call schedule provided by all covering surgeons.

The Coordinator assumes responsibility for the coordination of performance improvement activities in collaboration with the hospital quality and performance improvement department. The Coordinator will make decisions regarding activities in the area of responsibility based on Center of Excellence criteria in collaboration with the Program Medical Director, Performance Improvement Director, and Hospital Administration.

The Coordinator works closely with the Program Medical Director, program staff, physicians, patients, patient’s families, and the community to develop and manage strong relationships and business development activities including physician and community relations programs. Responsible for achieving goals and providing regular progress reports to Performance Improvement Director, and Executive leadership as needed.

Responsibilities Plan, coordinate and implement the overall Bariatric and Stroke programs. Plan and manage the care of patients, and family, as applicable.   Serve in accordance with the nursing process to continue follow-up and as a resource.

Serve as liaison between hospital, medical staff, clinical departments, bariatric physician’s office, patients and other resources.

Collaborates with interdisciplinary team members in the development, evaluation, and revision of the plan of care as appropriate to meet the patient’s needs and goals.

Performs concurrent review of patient care to ensure that protocols and care plans have been initiated and followed.

Collects and abstracts data into databases. Analyzes data to identify trends and new opportunities for process improvement. Documents findings and communicates all pertinent information to interdisciplinary team members as appropriate.

Participates in Regional Advisory Council (RAC) by attending meeting, organizing and facilitating the hospital’s involvement in the RAC.

Develop hospital relationships with primary and specialty practice physicians with the goal of increasing referral and redirection opportunities for both inpatient and outpatient services for the hospital.

Monitor patient experience and share information with leadership and other departments to identify opportunities to improve, resolve or prevent the recurrence of problems, and increase overall patient satisfaction.

Successfully plans and hosts Information Seminars and Support Groups monthly, in support of patient recruitment efforts and program continuing education.

Effectively encourages potential patients to complete pre-registration/insurance verification paperwork after Information Seminars/Support Groups, knowledgeably assists with this process, and verifies insurance eligibility.

Provides education to patients/families/significant other and staff, incorporating age-specific criteria and assessment of readiness to learn.

Supervises staff skills and knowledge verification.

Represents the hospital in a professional manner at community events as required.

 

Performs other duties as assigned or required.

 

EDUCATION, EXPERIENCE, TRAINING

 

  1. Current state licensure as a registered nurse, BSN preferred.
  2. Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certified.
  3. Five years clinical and/or management experience in a multidisciplinary health care setting which includes a minimum of two years of inpatient experience preferred.
  4. Knowledge on current State Laws and Federal Regulations and various accreditation requirements applicable to Bariatric and Stroke programs.
  5. Computer proficiency in Windows applications is required (especially Word, Excel, PowerPoint); strong working knowledge in data management, data analysis and quality improvement processes are required. Proficiency in Computerized Physician Order Entry (CPOE) and core clinical systems are required.
  • Public speaking experience in both large and small groups, preferred.
  • Demonstrated competencies include: Customer Focus, Problem-Solving, Strategy Development, and Performance Reporting.
