Job Description

1. Organizes, plans and conducts the therapeutic activity program, conducting a need assessment on a regular basis and designs the program based upon variances in the patient population.
2. In conjunction with the patient/family, develops an individualized patient activity program based on assessment/re-assessment of individual patient needs.
3. Develops daily programs for patients, including those patients not able to leave their rooms, which make life more meaningful and which support physical and mental capabilities.
4. Provides a variety of activities such as religious programs, creative activities, educational activities and physical exercises.
5. Requests and maintains needed equipment and supplies for the activity program.
6. Ensures patient and/or family involvement in the decision making process as it relates to patient activity plans, schedule and attendance.
7. Evaluates and documents the effectiveness of the patient/family teaching through assessment of their comprehension of the instruction and demonstration of appropriate responses.
8. Develops and maintains needed working relationships with community agencies and organizations for internal and external referral purposes.
9. Supports and participates in activities that foster customer service.
10. Demonstrates willingness and ability to float to areas within their scope of practice as necessary.
11. Performs all other duties as related or assigned.
