Overview

Summary: The Licensed Practical Nurse reports to the Clinical Manager. Responsible to provide nursing in a professional manner consistent with the Nurse Association Standards for Home Health Nursing.

 

Position Qualifications

Graduate of an accredited school of professional nursing. Current LPN license from the State in which he/she is working. Current CPR certification.

Minimum of two (2) years clinical experience in an acute-care setting or home care setting preferred.

Knowledge of current treatment modalities and possess the ability to assist with home visits.

Able to carry/lift 30-35 pounds, sit/stand 1-2 hours at a time

Maintain current CPR certification.

Valid drivers license and automobile insurance; reliable transportation

Successful completion of any additional educational requirements and knowledge of Agency Policies and Procedures.

 

Knowledge and Abilities

  • Demonstrate knowledge of the current environment of home health care and principles of customer service.

  • Ability to assess data reflecting resource utilization and referral source information.

  • Ability to operate computer.

  • Light to moderate travel to patient homes, seminars, inservices, etc

  • Position Duties

  • Assumes responsibility for receiving the necessary Continuing Education Units (CEUs) as mandated by the Board of Nursing.                    

  • Responsible for the following:

  •          a)                     Direct patient care and assessments

             b)                     Hands on patient care coordination with other nursing staff

             c)                     Following Plan of Care

             d)                     Ensuring Clinical Charts are up to Ohio Valley Home Health Services and Medicare standards.

             e)                     Inventory of patient supplies on weekly basis

             f)                     Delivery of patient supplies

             g)                    Communication with Clinical Managers/RN Case Managers of any clinical changes in patient’s condition. Relay significant changes in status to the physician when appropriate.

             h)                    Involve the patient/family in the establishment/implementation of the Plan of Care and realistic, individualized, and measurable patient goals.

             i)                      Evaluates patient progress toward goal achievement and makes modifications to the plan of care as needed.

             j)                      Participates in Case Conferences and collaborates/coordinates other agency disciplines as necessary.        

             k)                     Adheres to Ohio Valley Home Health Services, Inc. Policies and Procedures.

     

         3.  Provides Clinical report to any nursing provider (i.e. On-Call, weekend coverage, per diem, etc.) while covering their primary patient.

         4.  Must do patient visits after agency hours if scheduled personnel is unavailable.

         5.  Participates in monthly clinical record audits for PI program

         6.  Participates in billing audits as needed

         7.  Participates in chart audits as needed

         8.  Uses laptop computers for clinical documentation

         9.  Participates in Compliance Plan training if needed

       10.  Other duties as assigned.

     

    Responsibilities

    Responsible for assisting physicians with patient care, clerical, environmental and organization tasks. Responsible for appointment scheduling and chart management. Provides Information to patients so they may fully utilize and benefit from the clinic services. Conveys a positive image of organization.

     

    Qualifications

    EDUCATION, EXPERIENCE, TRAINING

     

  • High School Diploma or Equivalent or relevant experience required for new hires effective April 2014.

  • Licensed Practical Nurse (LPN) licensure.

  • Current BLS (AHA) certificate upon hire and maintain current

  • Minimum one year work experience in public contact job.

  • Work experience in health care preferred.
