Job Description

Summary: The Registered Nurse reports to the Clinical Manager. Responsible to provide nursing in a professional manner consistent with the American Nurse Association Standards for Home Health Nursing.
Position Qualifications
Must possess a Nursing (RN) degree from an accredited nursing program that is approved by an accredited school
Maintains a current licensure in the state in which he/she will be working.
Minimum of two (2) years clinical experience in an acute-care setting or home care setting preferred.
Demonstrate knowledge regarding home health care required.
Knowledge of current treatment modalities and possess the ability to assist with home visits.
Able to carry/lift 30-35 pounds, sit/stand 1-2 hours at a time
Maintain current CPR certification.
Valid drivers license and automobile insurance; reliable transportation
Successful completion of any additional educational requirements and knowledge of Agency Policies and Procedures.
Knowledge and Abilities
  • Demonstrate knowledge of the current environment of home health care and principles of customer service.
  • Ability to assess data reflecting resource utilization and referral source information.
  • Ability to operate computer.
  • Light to moderate travel to patient homes, seminars, inservices, etc.
  • Position Duties
  • Assumes responsibility for receiving the necessary Continuing Education Units (CEUs) as mandated by the Board of Nursing.
  • Maintains a caseload of primary patients.
  • Responsible for the following for primary patients:
             1)                     Initial patient assessments
             2)                     Hands on patient care coordination with other RNs
             3)                     Formulating Plan of Care (POC)
             4)                     Completion of 485/487s
             5)                     Discharging Clinical charts per Ohio Valley Home Health Services, Inc. policy
             6)                     Scheduling of visits on a weekly basis in conjunction with Scheduler
             7)                     Ensuring Clinical Charts are up to Ohio Valley Home Health and Medicare standards
             8)                     Inventory of patient supplies on weekly basis
             9)                     Delivery of patient supplies
            10)                    Communication with Clinical Managers/other RNs of any clinical changes in patient’s condition. Relay significant changes in status to the physician when appropriate.
            11)                    Involve the patient/family in the establishment/implementation of the Plan of Care and realistic, individualized, and measurable patient goals.
            12)                    Manages the course of care according to patient needs as determined by ongoing physical and psychosocial assessment and physician orders.
            13)                    Evaluates patient progress toward goal achievement and makes modifications to the plan of care as needed.
            14)                    Participates in Case Conferences and collaborates/coordinates other agency disciplines as necessary.
            15)                    Performs supervision of Home Health Aides. Completes Home Health Aide supervisory visits according to policy.
            16)                    Adheres to Ohio Valley Home Health Services, Inc. Policies and Procedures.
  • Provides Clinical report to any nursing provider (i.e. On-Call, weekend coverage,per diem, etc.) while covering their primary patient.
  • Must participate in on-call rotation per policy and procedure.
  • Must do patient visits after agency hours if scheduled personnel is unavailable.
  • Participates in monthly clinical record audits for PI program
  • Assists with billing audits
  • Participates in chart audits as needed
  • Uses laptop computers for clinical documentation
  • Participates in Compliance Plan training if needed
  • Other duties as assigned.
