Responsibilities

The Medical Case Manager will be responsible for providing direct case management to the patients of the Peter Ho clinic. The Medical Case Manager will support the Medical Case Management team with resources, training, and performs the various steps of the Medical Case Management including Intake, Assessment of Needs, Development of Service Plan, Implementation, Monitoring and Re-evaluation of the patients’ needs/discharge and termination of cases and support in the Medical Case Management functions.  Maintains documentation in the EMR, educates and consults with service providers and consumers concerning HIV disease and the process of meeting consumers’ identified needs.

Qualifications

  • Minimum of 5 years’ experience working in a health related field. 
  • Bachelors of Arts, Bachelors in Social Work or health care related field required.
